Business Travel Insurance Application

1. Only applicable to Business Travelsure Protection Plan.
2. Premium discount offer is only applicable to first policy year. A 35% discount will be offered and the premium for each insured employee is as low as HKD1,524 (original price: HKD2,345). For 2 or more insured employees, the premium for each insured employee is HKD1,492 (original price: HKD 2,295).
3. The amount of premium(s) above may slightly differ from the actual premium(s) payable by less than HKD 1.00 due to rounding differences.
4. Levy collected by the Insurance Authority ("IA") has been imposed on relevant policy (except for certain exempted insurance classes) at the applicable rate and would be remitted in accordance with the prescribed arrangements. 5. The premium above does not include levy collected by the IA, actual amount of the IA levy will be shown in confirmation page for this policy.
6. This insurance will, subject to the provisions in the Policy Schedule and Terms and Conditions of the Plan, be effective upon the issuance of the Policy Schedule. Once the Policy Schedule has been issued, you are obliged to settle all premiums due and no refund of premium is allowed.

1. This insurance quotation is only valid if (1) I/We am/are not traveling contrary to the advice of a Medical Practitioner, for the purpose of obtaining medical treatment, or for emigration; (2) the travel originates from Hong Kong SAR; and (3) the insurance policy is not accepted when black-rain warning or typhoon signal number 8 or above is hoisted; strikes or industrial actions of airport or airline staff are in progress or any other reasons known to me/us that the trip may have to be cancelled or re-scheduled during the time of application.
2. I/We have not withheld any material facts* (i.e. facts relevant to an insurer’s decision whether or not to provide coverage) from QBE General Insurance (Hong Kong) Limited and that if any material facts shall have been withheld or not truly or fairly stated, this insurance policy shall be null and void.

* If you are in doubt as to whether or not particular information or facts are material, they should be disclosed.

• This insurance is only valid for travel originating from Hong Kong.
• Cover period of up to 60 days for each trip (business or leisure).
• Unlimited number of trips within a year.
• The above offers cannot be used in conjunction with other offers of same type.
• QBE General Insurance (Hong Kong) Limited reserves the right to revise the terms and conditions of the offers or cancel the offers without prior notices to customer.
• Customer who holds or cancels or has held or cancelled any policy under the Business Travelsure Protection Plan in the past six months will not be entitled to the offers above.
• In case of any disputes, the decision of QBE General Insurance (Hong Kong) Limited and Hang Seng Bank Limited shall be final.
